Waiver
I have read the completed application, understand the rules of the Boys & Girls Club of Evansville, Inc. and request that _______________ (the “Child”) be admitted into membership. I have explained the rules to the Child. I understand it is my obligation to inform the Boys & Girls Club of Evansville, Inc. in writing of any limitation that causes the Child to be unable to safely participate in the program. In consideration for the privilege of participating in the program and other valuable consideration, I, on behalf of myself and the child, herby release, indemnify, hold harmless and discharge to the fullest extent permitted by lay the Boys & Girls Club of Evansville, Inc. from and against any claims, actions, demands, attorney and expert fees, damages, losses, costs, expenses and/or liabilities, whether such arises based negligence or by any statutory or common law theories, (Including without limitation all injuries, disability or death) arising out of or in connection with the program and even though that liability may arise out of the sole negligence or carelessness on the part of the Boys & Girls Club of Evansville, Inc. I hereby agree that the Boys & Girls Club of Evansville, Inc. shall not be liable for any injury or loss which myself or the child may sustain while participating in the program and we agree to indemnify and to hold harmless the Boys & Girls Club of Evansville, Inc. from any claim whatsoever including but not limited to reasonable attorney’s and expert fees. I give my consent for photographs or media exposure, in which the Child may appear, to be used in any way the Boys & Girls Club of Evansville, Inc. may care to use them. I grant permission for the Child to participate in any of the Boys & Girls Clubs of Evansville’s, Inc. surveys/program evaluations administered locally or nationally. I represent and warrant that I have the legal authority to enter into this agreement on behalf of the Child. Review Safety Policy & Procedures online or by email.
